Adults-Medical Rehabilitation Services

Orthopedic and Chronic Conditions

Courage Center offers physical therapy and aquatic physical therapy in a warm water pool at our locations in Golden Valley and Stillwater and we also partner to offer physical therapy at community sites in Coon Rapids, Burnsville, Plymouth and Eden Prairie. For clients with orthopedic and chronic conditions the goal is to increase independence. Courage Center has used the Pain Disability Index to assess gains made from physical therapy. The Pain Disability Index (PDI) is a short questionnaire used to measure the impact that pain has on the ability of a person to participate in essential life activities. A lower score on this measure indicates less limitation.

  • For those that took the PDI the median number of visits was5 and the average age was 57.
  • On average we saw a decrease in the limitation due to pain as measured by the PDI in clients with Back and Neck Pain or Injury (n=148), Arthritis, Degenerative Joint Disorder, and Fibromyalgia (n=70).

Courage Home and Community

Courage Center’s Home and Community Rehabilitation works with many disciplines toward increasing a person’s functional independence at home, work, school and in the community.  We bring our rehabilitation team to you, in your home or in community settings to reintegrate what you learn in therapy to your "real life".  Your therapy program is determined by your individual needs and potential. Each person is unique and goals are established with your input. Your team includes you, your family/concerned others, your physician, therapists, nurses, clinical coordinator and insurance case manager. Courage Residence (Transitional Rehabilitation Program)

Neurological Conditions

Licensed as a skilled nursing facility, our transitional rehabilitation program is like no other facility in this country. Our continuum of services supports participation and champions independence.


Clients Discharged to Community Living
  • The transitional rehabilitation program served 158 clients in 2006
  • The Median Length of Stay in the transitional rehabilitation program is 3.3 months.
  • The Average Age is 40.
  • 29 % Brain Injury, 36% Spinal Cord Injury, 15% Stroke, and 19% Congenital and Other

In 2006, Courage Center assisted 107 individuals to achieve and/or maintain employment. Courage Center’s successes in vocational service placement compare very favorably with Outcomes achieved by clients served through the State Vocational

Independent Living Skills

Independent Living Skills teach you how to strengthen your skills and do more for yourself—ultimately providing more independence in all areas related to community living. Services include: managing your household and finances; finding or maintaining affordable, accessible housing; developing a healthy lifestyle; identifying recreational and vocational opportunities and accessing the community.
